Anthropology Feedback Form

Welcome.

This form is intended for any feedback related to experiences in the Department of Anthropology, including issues related to department climate and culture, leadership, and policy.  It is not intended to report an emergency or critical situation.

Please be aware that if any person is named in feedback that relates to sexual misconduct or discrimination based on protected class, the information will be shared with the University of Colorado Boulder's Office of Institutional Equity and Compliance (OIEC). You can also report such information directly to OIEC by emailing cureport@colorado.edu or using the incident report form on their Don't Ignore It website. Reporting and resource options for a range of concerns can be found on the Don't Ignore It site. The CU-Boulder Office of Victim Assistance (OVA) and the Ombuds Office are campus resources for confidential advice that do not have an obligation to report to OIEC.

If you are providing feedback related to a course, please provide the course number or instructor name if you are comfortable doing so, as this information will assist us in addressing your concern.

Thank you for using this form to voice your comments or concerns relating to the CU Boulder Department of Anthropology. The information provided in this form will only be accessible by the Department Chair and Associate Chairs. If you would like your feedback to be anonymous, please do not provide any identifying information. However, if you like, you may provide your name so that the Anthropology Department can follow up with you as needed.
